Job summary

MaineHealth in Portland, ME, seeks a Registered Nurse - Patient Navigator for the PREP program. This role works with APPs and physicians to assess and optimize medically and socially complex patients and to coordinate preoperative care.

The RN Navigator provides proactive clinical support to close communication gaps, manage care gaps, and promote timely preoperative optimization to improve outcomes. BSN required; Masters preferred; Maine RN license required.

Qualifications

• Education: BSN degree required; Master's Degree preferred.
• License/Certifications: Current Maine RN license and ongoing education.
• Experience: Experience with preoperative patients and families preferred; community outreach experience preferred.

Responsibilities

• Coordinate care for preoperative patients and optimize readiness.
• Provide proactive clinical support to close communication gaps.
• Promote timely preoperative optimization in collaboration with APPs and physicians.
• Work with a small team to manage care gaps and coordinate pathways.
