Job summary

The Patient Navigation Supervisor at Maine Medical Center requests an experienced nurse leader to guide patients through the cancer care continuum, coordinate care across services, and support patients and families through diagnosis and survivorship.

This role emphasizes quality improvement, program evaluation, and collaboration with MMC Specialty Program staff to address barriers to access and ensure timely, coordinated care.

Qualifications

  • Education: BSN required; MSN preferred.
  • Current license to practice as a Registered Nurse in Maine; specialty certification preferred.
  • 8–10 years of clinical experience as staff nurse and/or supervisor; strong interpersonal/managerial skills.

Responsibilities

  • Guide patients through the cancer care continuum and establish contact with patients, families, or caregivers.
  • Direct patients to health care services within MMC, other facilities, and the community for timely care and survivorship.
  • Identify and address barriers to care to ensure timely diagnosis and treatment.
  • Monitor and evaluate the Navigator program metrics with support from the Director.

Summary

The Patient Navigation Supervisor is a vital link between the mission and vision of Maine Medical Center Cancer Institute and the delivery of safe patient and family centered care that is compassionate, high quality and evidence based. The Patient Navigator Supervisor is responsible for the following: guides patients through the care continuum, establishing contact with recurrent and newly diagnosed patients and/or family member, or caregiver; understands the clinical pathways established by the MMC Specialty Program and directs patients to health care services within the organization, at outside facilities, and within the community for timely diagnosis, treatment and survivorship; actively identify and address barriers to care that might keep the patient from receiving timely and appropriate treatment for their diagnosis; connects patients with resources, health care and support services in their communities and assist the patient and physicians in the transition from active treatment to survivorship; works closely with the MMC Specialty Program and community based service providers to identify and address community needs for screening services and education; is responsible for ongoing evaluation, quality improvement of the Specialty Navigator Program and monitoring of program metrics with support from Director.

Required Minimum Knowledge, Skills, and Abilities (KSAs)
  • Education: Bachelor of Science in Nursing. Masters in Nursing preferred.
  • License/Certifications: Requires current license to practice as a Registered Professional Nurse in the State of Maine. Specialty Nursing certification preferred.
  • Experience: Requires clinical expertise in direct patient care as normally acquired through approximately 8-10 years’ experience as a staff nurse and/or supervisor.
  • Requires interpersonal and management skills; knowledge of employee relations laws as normally acquired through the completion of BSN in order to deal with complex management problems and communicate with unit personnel, physicians, administrative personnel and a variety of other hospital personnel and community representatives.
  • Requires a high level of analytical skill to make management decisions at the unit level and make recommendations for consideration of departmental issues.
  • Demonstrated ability to train/teach preferred.

The pay range for this position is $89,980.80to $121,721.60per year.

Compensation for part‑time positions is determined on a prorated basis consistent with the scheduled hours and applicable full‑time equivalent (FTE).
