Job summary

Good Samaritan University Hospital, a member of Catholic Health, is seeking a Community Health Worker to identify high-risk prenatal and postpartum women and help them access primary and preventive health services on Long Island. You will support access to prenatal care, PCAP and WIC, using basic computer skills and clear written and verbal communication.

Spanish or Creole language skills are a plus; a NY driver's license is recommended.

Qualifications

• High School Diploma or equivalent.
• Valid New York State Driver's license, personal car to carry out employment duties and an active automobile insurance policy.
• Basic computer skills.
• Good writing and verbal skills.
• Spanish or Creole speaking a plus.

Responsibilities

• Identify high-risk prenatal and postpartum women.
• Assist these individuals in gaining access to primary and preventive health services including prenatal care, PCAP, WIC, and other essential and supportive services as needed.
