Job summary

Health and Welfare Council of Long Island (HWCLI) seeks an energetic Community Health Worker to support the Social Care Network under the 1115 Medicaid Waiver. You will screen Medicaid members, refer to services, and follow up to improve health outcomes.

Documentation will be in Unite Us and other systems as required. Reporting to the Director of Social Care and Navigation, this non-clinical role requires outreach by phone, video, or in person, with occasional travel across Nassau and Suffolk

Qualifications

• High school diploma or GED required.
• 2-3 years of relevant work experience.
• Experience in the community health care setting; health coach and/or patient navigator preferred.
• Valid Driver's License preferred and reliable transportation.
• Bilingual preferred.

Responsibilities

• Conduct screening and interviews with Medicaid members.
• Identify and verify eligibility using appropriate screenings.
• Verify demographic information in documentation platforms.
• Confirm client’s desire to receive social care services.
• Document consent and referrals in Unite Us and other systems.
• Perform outreach via virtual, telephonic, or in-person visits to assess needs and connect to resources.
• Develop care plans based on eligibility and needs.
• Monitor referrals and ensure services are provided.
• Identify and address barriers to referred services.
