Job summary

Good Samaritan University Hospital, part of Catholic Health, seeks a Community Health Worker to identify high risk prenatal and postpartum women and help them access primary and preventive health services including prenatal care, PCAP and WIC. The role requires a high school diploma, a NYS driver’s license, and strong communication skills.

Bilingual in Spanish or Creole is a plus and a personal vehicle is often needed.

Qualifications

• High School Diploma or equivalent is required.
• Valid New York State Driver's license; a personal car and active auto insurance are required.
• Basic computer skills are required; good writing and verbal skills are essential.
• Spanish or Creole speaking is a plus.

Salary Range

USD $22.05 - USD $22.05 /Hr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
