Qualifications

• Experience in prenatal or maternal health care setting.

Responsibilities

• Support primary care of patients under planned care model, including pre-visit planning and nurse-led visits.
• Triage of patients in person and by phone using clear protocols.
• Coordinate care with PCP and other community providers for preventive and chronic care.
• Administer medications, including immunizations, under provider orders and standing orders.
• Accurately document patient care in the medical chart and maintain infection control standards.
• Assist providers with exams, treatments, and prenatal NSTs.
