Job summary

AltaPointe Health is seeking a Community Support Professional to work with individuals served, implementing the Person-Centered Plan (PCP) within an IST framework. The role involves training, supervising, and coordinating with families and professionals to promote independent living and employment readiness.

Responsibilities include data collection on progress, health and incident reporting, and facilitating daily activities to enhance community inclusion.

Qualifications

• High school diploma or equivalent.
• Good oral and written communication skills.
• Ability to perform arithmetic operations accurately.
• Work may require lifting heavy objects (minimum 30 pounds).
• This position has driving responsibilities; valid Alabama driver’s license and proof of liability insurance required.

Responsibilities

• Instructs and interacts with people served to achieve goals per PCP.
• Collects data to assess progress per PCP.
• Serves as a link to community inclusion and plans daily activities.
• Reports health concerns and incidents to supervisor.
• Participates in quality/performance improvement activities.
