Job summary

AltaPointe Health Systems is seeking Case Managers to monitor and support consumers across mental health, physical health, living, learning, working, and social domains. Roles include traveling to homes, schools, and community living settings to assess needs and link to services.

You will work with treatment teams, document progress, support families, and help maintain connections with community resources while promoting recovery and independence.

Qualifications

• Bachelor's degree in a behavioral health or social services related discipline preferred.
• Must meet all driving requirements.
• Knowledge of psychiatric disorders in the adult population and/or addiction treatment concepts.
• Knowledge of cultural diversity and behavior management, crisis intervention.
• Knowledge of Psychiatric Rehabilitation principles and available community resources.

Responsibilities

• Assess consumer needs using SUN-R and document ISP goals.
• Link to community resources and monitor placement in least restrictive environments.
• Provide ongoing monitoring of progress and advocate for consumers.
• Collaborate with treatment team and outside sources in case management.
• De-escalate crisis situations and provide outreach to consumers and families.
• Train in basic living skills and assist with transportation to psychiatric appointments.
• Maintain thorough clinical records per AltaPointe standards.
• Participate in Performance Improvement activities and required committees.
