Job summary

LiveWell Homecare Agency seeks an experienced Registered Nurse to provide direct patient care in an outpatient setting and collaborate with physicians and the care team to implement individualized care plans. You will oversee clinical staff, support daily patient management, and assist providers with documentation and patient education.

Required RN licensure with a nursing diploma or associate degree and BLS certification.

Qualifications

• Diploma or Associate’s degree in Nursing; RN licensure.
• 2-5 years of nursing experience; supervisory experience not required.
• Certifications: BLS required.
• Preferred: Bachelor’s degree in Nursing and 5-10 years of experience.

Responsibilities

• Develop and update care plans with PCPs and specialists to enhance healthcare outcomes.
• Assess patients’ physical, psychosocial, and emotional needs using critical nursing skills.
• Ensure clinical protocols and assessment guidelines are followed for compliance and quality.
• Support documentation of patient assessments and treatments according to standards.
• Educate patients and families verbally and in writing, tailoring support to their needs.
• Work in a safety-sensitive environment, maintaining constant alertness and safety awareness.
