Job summary

Prava Therapy is seeking a Licensed Physical Therapist to own and lead a memory care therapy program inside a senior living community in Carlsbad, CA. This role combines clinical excellence with practice leadership and business operations.

You will partner with Prava to provide infrastructure, technology, and support while driving resident outcomes, engagement, and quality of life through a specialized memory care program.

Qualifications

• Licensed Physical Therapist in good standing.
• Experience in memory care or senior living preferred.
• Willingness to lead and own a practice.
• Full-time commitment to the role and community.

Responsibilities

• Lead the physical therapy program within a dedicated memory care community.
• Maintain a highly visible, consistent presence in the community.
• Evaluate and treat residents using strong clinical reasoning and individualized care plans.
• Develop meaningful relationships with residents, families, caregivers, and community leadership.
• Identify residents who may benefit from skilled therapy and develop appropriate referral pathways.
• Build a therapy culture focused on function, mobility, safety, participation, and quality of life.
• Collaborate with nursing, wellness, activities, physicians, families, and other care partners.
• Mentor and lead additional clinicians as the practice grows.
• Monitor clinical outcomes, utilization, documentation, and practice performance.
• Participate in Prava's operating rhythm, including weekly coaching and reporting.

We’re looking for a Physical Therapist Practice Owner to lead a thriving therapy program within a memory care community — someone who understands that exceptional therapy in memory care looks different, requires different clinical reasoning, and demands a deep understanding of the resident, the environment, and the people supporting them.

This is not a traditional PT job.

You won't be walking into a schedule and simply treating the next resident.

You will have the opportunity to build something.

As the Practice Owner, you will become the clinical leader and face of the therapy practice within the community. You'll develop relationships with residents, families, caregivers, nurses, physicians, leadership, and community partners while building a program that becomes an integral part of the community.
