Prava-Therapy is seeking a Part-Time Physical Therapist to join a Practice Owner in an Assisted Living and Memory Care community. This role emphasizes clinical care, relationships, and community presence within a therapist-owned model powered by Prava’s ecosystem.

You’ll provide meaningful PT focused on function, build strong relationships with residents and families, and collaborate with nursing, wellness, and leadership to enhance mobility and overall quality of life.

Qualifications

• Licensed Physical Therapist in the state of California.
• Experience with geriatric populations preferred.
• Ability to build relationships with residents and families.

Responsibilities

• Provide meaningful, function-focused PT in Assisted Living & Memory Care.
• Build relationships with residents, families, and care team.
• Partner with nursing and community leadership to support mobility and quality of life.
• Help residents maintain mobility, independence, and safety.
• Work with a Practice Owner to grow the practice within the community.
