Job summary

Ohio's Hospice of Central Ohio is seeking a Practice Manager to oversee the Palliative Care program, including referrals, scheduling, documentation and billing, reporting, and coordination with Care Team Coordinators. The PM will align operations with partners such as hospitals, facilities, and provider groups, and collaborate with Pure Healthcare on population health contracts.

The role requires leadership across departments, data-driven decision making, and ensuring high-quality patient care

Responsibilities

  • Collaborate across the organization, and uniquely with provider staff, to maintain quality driven, outcome measured Palliative Care in all settings.
  • Collaborate with physician leaders to provide evidence-based care in accordance with the organization's mission, vision and strategic objectives.
  • Contribute ideas and solutions directed toward an evolving model of Palliative Care
  • Understand, teach and reinforce protocols, systems, and processes.
  • Collaborate with Pure Healthcare to support at-risk populations and alternative payer arrangements to achieve target clinical outcomes.
  • Utilize clinical, operational, patient satisfaction, and other data to improve systems of care.
  • Collaborate with physician leaders to support high-quality patient care, safety, accurate and timely documentation.
  • Collaborate with the Provider Credentialing and Support Specialist to support provider scheduling and completion of duties
  • Create, follow and communicate reports specific to populations served.
  • Understand and implement written policies and procedures.
  • Evaluate provider productivity to maintain healthy workforce and organizational growth.
  • Evaluate customer satisfaction and contribute to the resolution of customer service concerns.
  • Assist in other activities and departments when requested.

Location: Must be based within one of the following regions:

Job Summary/Purpose - The Practice Manager (PM), Palliative Care provides oversight of OHI's Palliative Care Program, to include referrals, scheduling, documentation, billing, coding, quality reporting and other operations. The PM is supported by and supervises the Care Team Coordinators (CTCs). The PM ensures operational alignment with contracted and non-contracted community partners to include hospitals, facilities and provider groups. The PM will collaborate with Pure Healthcare in the support of population health contracts, navigating processes to achieve successful outcome measurements.

Qualifications-
  • Minimum of five years medical practice management experience.
  • Bachelor's Degree in business administration, healthcare administration or related field
  • Excellent written and verbal communication skills
  • Computer skills sufficient to properly document services and analyze data including Microsoft Office 365, Teams, Power Point
  • Valid Driver's License with Safe Driving Record
  • State Minimum Automobile Insurance Coverage
