Job summary

Ohio's Hospice, Inc. in Dayton, OH is seeking a Hospice RN Care Manager to deliver direct patient care this full-time, Monday–Friday, 8am–5pm, with every 16th weekend rotation and one holiday per year.

You will plan and deliver care to patients using the nursing process, coordinate with families and an interdisciplinary team, and support patients and their loved ones on the end‑of‑life journey. Qualifications include Ohio RN licensure, ASN/BSN (BSN preferred), CPR and CHPN preferred (required

Qualifications

• RN licensure in the State of Ohio.
• ASN with BSN preferred; CHPN preferred and required within two years.
• Two years of acute care nursing experience preferred.

Responsibilities

• Plan and deliver care to patients using the nursing process (assessment, planning, interventions, implementation, evaluation).
• Collaborate with patients, families and an interdisciplinary care team.
• Document services accurately and maintain standards of professional nursing.
• Drive to patient locations as needed and maintain a valid driver's license.

Hospice RN Care Manager FT Days

Full Time Direct Patient Care Dayton, OH, US

Hospice RN – Making every moment matter

• This is a full-time M-F 8am-5pm position with every 16th weekend rotation and 1 holiday requirement per year. You will be delivering care to patients living in facilities and homes with the greater Huber Heights and surrounding areas.

As a Hospice RN you will plan and deliver care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interact with patients, families and other interdisciplinary team members while maintaining standards of professional nursing. We provide superior care and superior services to patients at their end‑of‑life journey. Only those who have a heart for hospice will succeed.

Hospice RN Qualifications:

RN's – Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred. Active Registered Nurse license in the State of Ohio without any board actions. Two years of acute care nursing experience preferred.

• CPR Certified
• Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
• Computer skills are sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record
• State Minimum Automobile Insurance Coverage
