Job description

About Seen Health

At Seen Health, we are revolutionizing the way senior care is delivered through the PACE (Programs of All-Inclusive Care for the Elderly) model. Backed by top VCs, Seen Health is a culturally-focused, technology-enabled healthcare organization that integrates comprehensive medical care and social support with a high-touch, interdisciplinary approach.

Our mission is to empower seniors to age-in-place with dignity and provide their families peace of mind. We are building upon a proven Home and community based services model to create a culturally-competent and scalable PACE program. We are also building a comprehensive operating system focused on data and workflows that span across systems, processes, people, and care contexts. We want to empower our clinicians and staff with tools that deliver relevant data at the time and site of care and enable them to deliver exceptional care to our participants, which improve clinical outcomes, participant & provider satisfaction, and ultimately our strength as an organization.

We are a mission-driven, multidisciplinary team with deep healthcare, technology, and operations expertise, each inspired by our own personal stories of caring for seniors in our lives. Our name, Seen Health, was chosen to reflect our commitment to provide the highest standard of care to underserved older adults while respecting and incorporating their individual beliefs, heritage, and values, so that they can truly be seen .

About the Role

Seen Health provides care for older adults in the San Gabriel Valley, many of whom have complex health needs. As Practice Manager, you'll run our clinic day to day so every participant gets timely, coordinated care in a language and style that feels familiar to them. You'll lead our clinical team of RNs, LVNs and Medical Assistants, building the schedules, workflows and habits that hold up on the busiest days. When the practice runs well, our clinicians can focus on patients, and participants and families feel the difference at every visit. You'll report directly to our Director of Clinical Operations, a true partner in this work, and together you'll own how well our clinic runs and the quality of care our participants receive. If you love building teams, improving how things work, and seeing your impact on the people we serve, we'd love to meet you.

Clinic Operations
  • Oversee daily operations of the medical office, including patient flow, scheduling, check-in/check-out, rooming, referrals, and follow-up

  • Manage supplies, equipment, and vendor relationships; ensure the clinic environment is clean, safe, and senior-friendly (accessibility, mobility aids, wayfinding)
Workflow Optimization
  • Evaluate existing clinical and administrative workflows, identify bottlenecks, and design, implement, and sustain improvements
  • Standardize processes such as rooming protocols, pre-visit planning, orders and results management, referrals, and care transitions
  • Use data and frontline feedback to measure impact and refine processes continuously
  • Lead the team in adopting new technology and AI tools, fostering an AI-first mindset to streamline daily workflows and improve operational efficiency
Quality, Compliance & Safety
  • Ensure compliance with applicable federal, state, and local regulations, including HIPAA, OSHA, CLIA, infection control, and CMS/DHCS requirements

  • Ensure communication with participants and caregivers is clear, respectful, and delivered in their preferred language
  • Promote a welcoming environment that reflects the cultural needs of Chinese-speaking seniors
Collaboration & Reporting
  • Work closely with physicians, advance practice providers, social workers, therapists, transportation, and other interdisciplinary team members
