Job summary

Seen Health, Inc. is hiring a Clinic Licensed Vocational Nurse (LVN) in the Los Angeles area to collaborate with a clinic team serving geriatric patients, including substantial patient panels with Primary Care Providers.

The role welcomes experienced LVNs and new graduates who hold a California LVN license. You will perform triage, wound care, vaccinations, and medication administration, and support Home Care visits as part of a holistic, interdisciplinary care model aimed at aging in place with

• Assist with post-appointment processes (diagnostic orders, labs, referrals).
• Educate participants and families on care plans, chronic disease management, and preventive care.
• Conduct health assessments and help develop care plans.
• Assist with home care visits and safety assessments when needed.

About The Role

As a Clinic Licensed Vocational Nurse (LVN), you will collaborate with a dedicated clinic team to support a substantial patient panel alongside Primary Care Providers (PCPs), ensuring high-quality care and positive health outcomes. This role is ideal for experienced and new graduate LVNs who have obtained their license, providing a unique opportunity to gain hands‑on experience in a clinic setting serving the geriatric population of San Gabriel Valley.

Responsibilities

• Assist with triage of inbound communications, including calls, faxes, emails, and instant messages from teammates, patients, and partners, prioritizing patient needs and coordinating care accordingly.
• Administer basic clinical procedures, such as wound care, vaccinations, and medication administration, within the LVN scope of practice.
• Provide on-site instruction and support to Personal Care Assistants/CNAs as needed, under the direction of the Clinic Manager and RNs.
• Manage post-appointment processes, including assisting with diagnostic orders (POC testing, labs), medication management, and referrals.
• Reinforce education for participants and their families on care plan, chronic disease management, and preventive care strategies.
• Conduct participant health assessments and assist in the development of care plans.
• Provide assistance with home care visits and participate in home safety assessments under supervision, if needed.

Qualifications

• Bilingual proficiency in English and spoken Mandarin and/or Cantonese.
• Graduate of an accredited School of Nursing (LVN program) required.
• Unencumbered LVN license in the state of California.
• Reliable means of transportation for potential home visits.
• Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations.
• Background in urgent care, emergency department (ED), long-term care, or a doctor's office setting is highly valuable.

Location

• Los Angeles required. Ability to commute to Alhambra required.

• Salary: $35.00 – $45.00/hr
