Job summary

Seen Health is seeking a Clinic LVN to support a clinic team serving the geriatric population in the San Gabriel Valley.

The LVN will triage communications, administer basic procedures, and assist with care plans under supervision of RN and Clinic Manager, contributing to high-quality outcomes and patient satisfaction.

Qualifications

• Graduate of an accredited School of Nursing (LVN program) required.
• Unencumbered LVN license in the state of California.
• Reliable means of transportation for potential home visits.
• Nursing knowledge and skills for frail, elderly participants; experience in urgent care/ED/long-term care valuable.

Responsibilities

• Triage inbound communications and coordinate care.
• Administer basic clinical procedures within LVN scope (wound care, vaccinations, medications).
• Provide on-site instruction to CNAs under supervision of Clinic Manager and RNs.
• Manage post-appointment processes, including diagnostics, labs, medications, and referrals.
• Reinforce education on care plans, chronic disease management, and preventive care.
• Conduct participant health assessments and assist in care plan development.
• Assist with home care visits and home safety assessments as needed.

About the Role

As a Clinic Licensed Vocational Nurse (LVN), you will collaborate with a dedicated clinic team to support a substantial patient panel alongside Primary Care Providers (PCPs), ensuring high-quality care and positive health outcomes. This role is ideal for experienced and new graduate LVNs who have obtained their license, providing a unique opportunity to gain hands-on experience in a clinic setting serving the geriatric population of San Gabriel Valley.

Responsibilities

• Assist with triage of inbound communications, including calls, faxes, emails, and instant messages from teammates, patients, and partners, prioritizing patient needs and coordinating care accordingly.
• Administer basic clinical procedures, such as wound care, vaccinations, and medication administration, within the LVN scope of practice.
• Provide on-site instruction and support to Personal Care Assistants/CNAs as needed, under the direction of the Clinic Manager and RNs.
• Manage post-appointment processes, including assisting with diagnostic orders (POC testing, labs), medication management, and referrals.
• Reinforce education for participants and their families on care plan, chronic disease management, and preventive care strategies.
• Conduct participant health assessments and assist in the development of care plans.
• Provide assistance with home care visits and participate in home safety assessments under supervision, if needed.

Qualifications

• Bilingual proficiency in English and spoken Mandarin and/or Cantonese.
• Graduate of an accredited School of Nursing (LVN program) required.
• Unencumbered LVN license in the state of California.
• Reliable means of transportation for potential home visits.
• Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations.
• Background in urgent care, emergency department (ED), long-term care, or a doctor’s office setting is highly valuable.

Location

• Los Angeles required. Ability to commute to Alhambra required.
