Affinity Hospice Care in California seeks an experienced Licensed Practical Nurse (LPN/LVN) to provide direct patient care in home settings under RN supervision. Per diem position with flexible scheduling, focusing on compassionate, patient-first care in patients' homes.

Requirements include active California LPN/LVN license, 3+ years nursing experience (home health preferred), CPR certification, and a valid driver’s license with reliable vehicle for travel to patient homes.

• : Current CPR certification with AHA.
• : Valid driver’s license and reliable insured vehicle.

Responsabilidades

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
• Convert to Full-Time Based On Performance If Interested

Please note that this role is per diem with a rate of $50 - $60 per visit.
