• Assesses patient/family needs using nursing process.
• Coordinates multidisciplinary plan of care.
• Delivers patient education and discharge planning within standards.

Responsibilities

• Use independent professional judgment, analytical skills, and the nursing process to provide a full range of delegated, interdependent, and independent nursing services to patients.
• Provide patient care that reflects respect for patient rights, dignity, values, and education needs.
• Collaborate with hospital staff and case managers to develop discharge plans.
• Monitor patient responses and adjust care in real time.
• Document nursing interventions and outcomes on shift workflows and POC.
