Qualifications

• 1 year Nursing experience.

Responsibilities

• Collects pertinent assessment data from patient, family, and healthcare team; interprets data to identify needs and problems.
• Develops, reviews, and revises an individualized care plan with measurable outcomes.
• Evaluates patient response to care and documents changes.
• Delivers care using nursing process and documents teaching and learning.
• Prepares for discharge/transfer and documents processes.
