Qualifications

Responsibilities

• Collects pertinent assessment data from patient and care team.
• Develops and revises an individualized care plan with measurable outcomes.
• Evaluates patient responses and documents changes in condition.
• Uses nursing process to teach and document patient education.
• Prepares for discharge/transfer and documents the process.
• Delivers care following pathophysiology and nursing principles.
• Recognizes patient age in assessment and care decisions.
• Communicates effectively with patients, families, and staff.
• Maintains complete and accurate charting and handoffs.
