Job summary

MaineHealth is seeking a compassionate Post Transplant Coordinator (PoTC) to join our multidisciplinary transplant team. You will coordinate and manage patient care after solid organ transplantation, serving as a vital link between recipients, nephrologists, surgeons, and the care team.

The PoTC collaborates with the transplant team within the MaineHealth Transplant Program Portland, ensuring continuity of care and delivering high-quality, patient-centered services across the transplant journey.

Qualifications

• Education: Bachelors Degree preferred.
• License/Certifications: Maintains licensure and certification respective of profession.
• Experience: RN, PA, NP, recent critical care, or other related experience required.

Responsibilities

• Coordinate and manage patient care following solid organ transplantation, serving as a vital link between transplant recipients, transplant nephrologists, surgeons, and other members of the care team.
• Ensure continuity of care across the transplant journey, provide patient education, monitor clinical progress, coordinate follow-up services, and promote outcomes for transplant recipients.
