Qualifications

• BSN required.
• Current State of Illinois Registered Professional Nurse license.
• American Heart Association BLS certification.

Responsibilities

• Collects and analyzes data, identifies outcomes, develops and implements an individualized plan, coordinates care delivery and evaluates progress toward outcomes.
• Directs care and determines appropriate utilization of any assistant involved in direct patient care.
• Collaborates with patient, family, physicians and staff; documents a plan focused on outcomes and communication.
• Participates in shared governance, committee work and unit projects.
• Evaluates and documents patient responses to nursing interventions in the health record and personalizes the plan of care based on evaluation findings.
• Utilizes the best available evidence, including research findings to guide practice decisions.
