Job summary

Specialized Education Services, Inc. (SESI) is seeking a Transition Coordinator to support students aged 14 and up as they transition from high school to post-secondary options.

You will collaborate with special education teachers, social workers, and administrators to plan job and life skills training, community-based instruction, and college and career readiness. This role emphasizes individualized supports, data-driven progress tracking, and strong engagement with families and school teams to

Qualifications

• Bachelor’s degree is required.
• Valid teaching credential appropriate to the role is preferred (secondary special education).
• Commitment to learning and implementing feedback.
• Adaptability and flexibility.

Responsibilities

• Diagnose and address learning, social, and emotional skill gaps for students.
• Collaborate with school teams and families to provide guidance and deliver SESI’s evidence-based models and PBIS framework.
• Develop and maintain individualized skill-appropriate lesson plans.
• Implement behavior management and intervention strategies, including de-escalation, crisis management, and safe restraint when necessary.
• Maintain student documents such as IEPs, BIPs, incident reports, and high-risk evaluation forms.
• Drive, record, and track student growth toward academic, social, and emotional goals.
• Supervise and escort students to and from locations within the school.
