Job summary

Children and Families First Delaware Inc in Delaware is seeking a Foster Care and Adoption Post-Permanency Case Manager. Based in Wilmington, you will provide case management, advocacy, and youth mentoring for families connected through foster care, adoption, or guardianship.

You will also lead support groups and training for foster and adoptive parents, maintain compliant records, and participate in court proceedings as needed.

Qualifications

• Bachelor's degree in Social Work, Human Services, or a related field is required.
• Previous experience in foster care, adoption, or human services is preferred.
• Excellent written and verbal communication skills.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Provide comprehensive case management, advocacy, and support to birth families, resource parents, and youth.
• Lead group sessions and training for foster and adoptive families.
• Maintain records to meet agency, state accreditation, and policy requirements.
• Complete court updates and testify in court when required.
• Collaborate with agency and community partners to coordinate services.
