Job summary

St. Mary’s Community Services is seeking a Recuperative Care Specialist to support clients transitioning from hospital care. You will monitor health, coordinate follow-ups, and assist with meals, transportation, and daily care needs while maintaining documentation and HIPAA compliance.

You will promote health literacy and provide trauma-informed care to medically vulnerable individuals experiencing homelessness, ensuring safe recovery and continuity of care.

Qualifications

• High school diploma or GED required.
• Medical Assistant Certificate (MA) required; CNA/EMT preferred.
• 1–2 years of healthcare, recuperative care, shelter, or similar direct client care.
• Valid California driver’s license and proof of personal liability insurance.
• Ability to operate a company vehicle and coordinate medical appointments.
• Bilingual English/Spanish preferred.

Responsibilities

• Perform initial intakes and document findings for discharge planning.
• Assess vitals, schedule follow-ups, and handle prescriptions as needed.
• Conduct morning/afternoon/evening checks on clients post-discharge.
• Coordinate home health visits and transportation to appointments.
• Provide health education and trauma-informed care to clients.
• Maintain HMIS and medical logs; ensure HIPAA compliance.
