RiverPointe Post-Acute Rehab in Carmichael, CA is seeking a Social Service Assistant to support residents in managing physical, mental and psychosocial needs under supervision. You will participate in admissions, coordinate care items, and communicate with families and physicians to support care plans.

Qualifications

• High School diploma or equivalent.
• Prior experience in social services, social work, healthcare, or long-term care setting preferred.
• SSD Certificate enrollment within 6 months of employment.

Responsibilities

• Assist with admission processes, including tours and interviews with residents, families or responsible parties.
• Arrange transportation, personal care items, adaptive equipment and other needs.
• Maintain contact with family members and physicians for care plan communications.
• Document interactions and referrals in the social services log.
• Support in grievance resolution and handling loss issues as requested.
