Qualifications

Responsibilities

• Assist the Social Services Director with residents' needs.
• Welcome new admissions and help residents adjust.
• Provide support throughout residents' stay.
• Assist with discharge planning and coordination.
• Schedule appointments and coordinate transportation.
• Help residents with paperwork and documentation.
• Participate in interdisciplinary care plan meetings.
• Support resident rights and quality of life initiatives.
