Job summary

Saint Francis Health System in Muskogee, OK is seeking a Rehab Admissions Liaison to coordinate entry into post-acute programs. The role requires evaluating patients for medical appropriateness, educating patients and families, and processing admissions referrals with insurance verification.

RN preferred with a minimum of 3 years bedside/related experience in inpatient rehabilitation. Responsibilities include collaboration with case managers, physicians, and payers to optimize post-acute care

Qualifications

• Must be a graduate of an accredited Respiratory Care, Occupational Therapy, or Physical Therapy Assistant program; or have completed the basic professional curricula of a nursing program as approved and verified by a state board; or hold a master’s degree in social work.
• Valid Oklahoma Provisional/Respiratory Care Practitioner License (RCP) or NBRC credentials, or valid State RN license, or State Social Work license.
• Minimum 3 years of related experience.
• Knowledge of post-acute services and third-party payer reimbursement.
• Ability to handle complex situations with patients, families, physicians and insurers.

Responsibilities

• Coordinates the point of entry to post-acute programs and assesses medical necessity.
• Obtains and verifies patient insurance benefits and processes referrals.
• Educates patients, families, and clinicians on post-acute services and criteria.
• Participates in case management meetings, marketing efforts, and census development.
