Job summary

PACS Group is seeking a dedicated Physical Therapist to evaluate and treat residents at our San Jacinto Valley Post Acute location in Hemet, CA. You will develop individualized plans to improve mobility, reduce pain, and restore function in collaboration with an interdisciplinary team.

Responsibilities include ongoing assessment, patient education, and documentation to ensure high-quality, patient-centered care that aligns with facility standards and regulatory requirements.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current California Physical Therapist license or eligibility.
• CPR certification preferred.
• Experience in skilled nursing, rehabilitation, outpatient, or post-acute care preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans based on clinical findings and goals.
• Provide therapeutic interventions to improve strength, mobility, balance, endurance, and functional independence.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nursing staff, therapy team members, residents, and families to ensure coordinated care.
• Educate patients and caregivers on treatment plans, exercises, and strategies to support recovery and long-term wellness.
• Maintain accurate and timely documentation in accordance with regulatory and facility requirements.
• Ensure compliance with all applicable professional, ethical, and safety standards.
