Waterman Canyon Post Acute seeks a Physical Therapist (PT) to evaluate and treat patients, develop plans, and collaborate with an interdisciplinary team in a post-acute care setting.

You will educate patients and families on exercises and recovery strategies, maintain compliant documentation, and contribute to a positive therapy environment. A license in good standing is required; SNF/post-acute experience is preferred. Full-time with competitive hourly pay.

Formación

• Current Physical Therapist license in good standing.
• Experience in a skilled nursing or post-acute setting preferred.
• Strong clinical judgment and communication skills.
• Patient-focused mindset with a passion for functional outcomes.
• Ability to work independently and as part of a collaborative team.

Responsabilidades

• Evaluate and treat patients to improve mobility, strength, balance, and overall function.
• Develop and implement individualized treatment plans.
• Collaborate with an interdisciplinary care team to support patient-centered goals.
• Educate patients and families on therapeutic exercises and recovery strategies.
• Maintain accurate and timely documentation in accordance with regulatory standards.
