Job summary

TeamHealth is seeking a quality-driven Nurse Practitioner or Physician Assistant to join our post-acute care team in Evansville, Indiana. This full-time role (5 days/week) involves rounding in skilled nursing facilities with strong support, training, autonomy, and earning potential.

You will provide comprehensive care to a diverse patient population and work with an experienced multidisciplinary team. The onboarding program includes a 3-month orientation and clinical mentorship.

Qualifications

Requires NP or PA license (Indiana) and DEA.
Post-acute, acute, ED, or clinic experience preferred but not required; new grads welcome.

Responsibilities

Perform comprehensive patient assessments (history & physical exams).
Develop, implement and adjust individualized care plans.
Monitor progress and coordinate care to optimize outcomes.
Collaborate with physicians, nursing staff and other professionals as a team.
Educate patients and families on health management and preventive strategies.
Maintain accurate, timely medical records in compliance with regulations.
