Job summary

Theoria Medical in Evansville, IN is seeking a full-time Advanced Practice Provider to deliver high-quality care for residents in skilled nursing facilities. You’ll perform daily rounding, manage treatment plans, and collaborate with a multidisciplinary team to ensure consistent, patient-centered care.

Qualifications

• Active U.S. license and board certification by ANCC/AANPCB/PNCB/AACN (for NPs) or NCCPA (for PAs).
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Geriatric Medicine.
• Experience in post-acute/SNF settings preferred.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounds, prescribe medications and treatment plans.
• Address acute and emergent issues and coordinate with facility staff.
• Maintain reliable on-site attendance for scheduled shifts and ensure regulatory compliance.
