Job summary

Enhabit Home Health & Hospice seeks a dynamic professional to manage relationships with physicians, hospitals, and health systems. You will educate referral sources on Enhabit services, supporting post‑acute care transitions to improve patient outcomes and satisfaction.

The role requires experience in healthcare business development, strong communication, and the ability to work onsite to collaborate with care teams and leadership. A valid driver license is required.

Qualifications

• Bachelor's Degree in healthcare administration, marketing, or related field or nursing/therapy/social work graduate.
• Highly preferred graduate of nursing, therapy, or social work.
• Minimum two years in healthcare business development, community relations, marketing, or sales.
• Knowledge of federal/state/local laws governing Medicare home health and hospice operations.
• Valid state driver license and automobile liability insurance.
• Strong interpersonal, communication and presentation skills.

Responsibilities

• Serve as primary relationship manager between Enhabit and referral sources (physicians, hospitals, health systems, senior living communities).
• Grow referrals by educating partners on Enhabit services and program requirements to support post‑acute care transitions.
• Navigate care transitions to improve patient outcomes and satisfaction.
