CHRISTUS Health is seeking a Post Acute Care Supervisor to support the Team Leader in the vision for the ACO/CIN. This role oversees daily operations of post-acute programs, ensuring high-quality patient care and efficient workflows.

You will lead nursing staff, develop programs to reduce post-acute spend, and implement policies to maintain care standards while advocating for patients’ preferences in care planning and decision-making.

Competenze

• BSN preferred.
• 4 years post-acute nursing experience, including home health, SNF, inpatient rehab, or LTACH.
• 2 years in a supervisory role.
• 5 years supporting value-based care programs, accountable care organizations, or utilization management preferred.
• RN license in the state of employment or compact is required.

Mansioni

• Oversee daily operations of post-acute programs for the ACO/CIN.
• Lead nursing staff and mentor associates to foster professional growth.
• Monitor post-acute spend and establish goals.
• Develop policies and procedures and ensure HIPAA compliance.
• Collaborate with manager and director on opportunities for improvement.
