Job summary

MedCap Health is seeking a full-time Post-Acute Care and Long Term Care NP to join our physician-led team. You will provide high-quality management for patients with chronic diseases, collaborating with the attending physician and interdisciplinary staff.

Work is day shift, Monday to Friday, with no overnights, offering autonomy, flexible scheduling, and opportunities for growth within a rapidly expanding network across multiple facilities.

Qualifications

• Board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP).
• 1+ years of post-acute/SNF/LTC experience.
• Prior post-acute, long-term, and/or skilled nursing facility experience.

Responsibilities

• Provide high-quality management for patients with chronic diseases.
• Collaborate with attending physician and interdisciplinary team to coordinate care.
• Communicate with Director of Nursing, social worker, and other staff to improve patient care.

In this role, your expertise will have a tangible impact on patient outcomes every day. You will be responsible for providing high-quality, effective management for patients with debilitating chronic diseases. Working closely with the attending physician, you manage acute and chronic conditions, ensuring seamless and comprehensive care. You are an integral part of the interdisciplinary team, communicating and collaborating regularly with the Director of Nursing, social worker, and other key staff to coordinate and improve patient care. Your contributions are vital, and you'll know that you are making a meaningful difference.

REQUIREMENTS

• A board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
• 1+ years of experience in post-acute/SNF/LTC
• Prior post-acute, long-term, and/or skilled nursing facility experience
