Job summary

MedCap Health in Tulsa is seeking a Post-Acute and Long-Term Care APP to help us build a program from the ground up, delivering high-quality care to patients in post-acute and long-term settings. This contract role offers $150,000 to $200,000 annually, with a benefits package including medical and dental coverage, paid time off, and a 401(k).

You can choose full-time, part-time, or flexible day shifts (Monday–Friday, no nights).

Qualifications

• 1+ years of experience in post-acute/SNF/LTC.
• Board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP).
• Post-acute, long-term, and/or skilled nursing facility experience.

Responsibilities

• Providing high-quality care for patients managing chronic and acute conditions.
• Working alongside an attending physician to guide treatment decisions.
• Partnering with Directors of Nursing, administrators, and care teams to coordinate patient care.
• Helping establish strong clinical practices as MedCap Health enters the community.

WHAT WE NEED FROM A POST-ACUTE AND LONG-TERM CARE APP

• 1+ years of experience in post-acute/SNF/LTC
