MedCap Health is seeking a Post-Acute Care and Long Term Care NP (full-time/part-time/flexible) to lead patient management in post-acute and long-term care settings. You will work closely with attending physicians and an interdisciplinary team to deliver comprehensive care across SNFs and LTC facilities.

Qualifications

In this role, your expertise will have a tangible impact on patient outcomes every day. You will be responsible for providing high-quality, effective management for patients with debilitating chronic diseases. Working closely with the attending physician, you manage acute and chronic conditions, ensuring seamless and comprehensive care. You are an integral part of the interdisciplinary team, communicating and collaborating regularly with the Director of Nursing, social worker, and other key staff to coordinate and improve patient care. Your contributions are vital, and you'll know that you are making a meaningful difference.

REQUIREMENTS

• A board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
• 1+ years of experience in post-acute/SNF/LTC
• Prior post-acute, long-term, and/or skilled nursing facility experience
