Job summary

MedCap Health in Tulsa is seeking a full-time Post-Acute Care and Long Term Care APP to join a physician-led team. You will provide high-quality management of chronic and acute conditions with autonomy and flexible scheduling, while maintaining a strong commitment to patient care.

Collaborating with the interdisciplinary team, including nursing and social work, you will coordinate comprehensive post-acute care and long-term services, ensuring optimal patient outcomes in a supportive environment.

Qualifications

• Board-certified Nurse Practitioner with specialization in geriatrics, family medicine, or adult primary care.
• 1+ years of post-acute/SNF/LTC clinical experience.
• Experience in post-acute, long-term, or skilled nursing facilities preferred.

Responsibilities

• Provide high-quality management of patients with chronic and acute conditions.
• Collaborate with attending physicians and interdisciplinary team to coordinate care.
• Support autonomy and participate in care planning and patient education.

YOUR SCHEDULE

Our schedule is designed to promote a healthy work-life balance, ensuring you have ample time for yourself. You'll work the day shift, Monday to Friday, with no nights.

In this role, your expertise will have a tangible impact on patient outcomes every day. You will be responsible for providing high-quality, effective management for patients with debilitating chronic diseases. Working closely with the attending physician, you manage acute and chronic conditions, ensuring seamless and comprehensive care. You are an integral part of the interdisciplinary team, communicating and collaborating regularly with the Director of Nursing, social worker, and other key staff to coordinate and improve patient care. Your contributions are vital, and you'll know that you are making a meaningful difference.

REQUIREMENTS

• A board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
• 1+ years of experience in post-acute/SNF/LTC
• Prior post-acute, long-term, and/or skilled nursing facility experience
