MedCap Health is seeking a Part-Time/Full-Time/Flexible APP to join our physician-led team in post-acute and long-term care. You will work daytime hours Monday–Friday with no overnights, providing comprehensive management for patients with chronic diseases in collaboration with physicians and an interdisciplinary team.

Qualificações

• Board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP).
• 1+ years of experience in post-acute/SNF/LTC.
• Prior post-acute, long-term, and/or skilled nursing facility experience.

Responsabilidades

• Provide high-quality management for patients with chronic diseases in post-acute settings.
• Collaborate closely with attending physician and interdisciplinary team to coordinate care.
• Communicate with Director of Nursing and social workers to improve patient outcomes.

YOUR SCHEDULE

Our schedule is designed to promote a healthy work-life balance, ensuring you have ample time for yourself. You'll work the day shift, Monday to Friday, with no nights.

In this role, your expertise will have a tangible impact on patient outcomes every day. You will be responsible for providing high-quality, effective management for patients with debilitating chronic diseases. Working closely with the attending physician, you manage acute and chronic conditions, ensuring seamless and comprehensive care. You are an integral part of the interdisciplinary team, communicating and collaborating regularly with the Director of Nursing, social worker, and other key staff to coordinate and improve patient care. Your contributions are vital, and you'll know that you are making a meaningful difference.
