Responsibilities

Complete an individualized functional behavior assessment.
Develop a person-centered, positive-practice, individualized behavior support plan and/or positive support transition plan with specific strategies.
Distribute the plan(s) to the person and team members.
Evaluate the effectiveness of the service and interventions.
Modify the plan(s) as necessary.
Provide on-site instructional learning to staff and caregivers about positive support.
Train and supervise Positive Support Specialist.
Observe and assess clients’ behavior.
Monitor and record treatment progress.
Develop and implement treatment plans to address behavioral issues.
Support clients throughout the treatment process.
Maintain patient records and treatment reports.
Collaborate with other healthcare specialists and support organizations.
