Responsibilities

Conduct comprehensive psychosocial assessments to identify member needs and barriers.
Develop individualized care plans with members, caregivers, providers, and the care team.
Coordinate referrals to behavioral health services and community resources.
Monitor member outcomes and support transitions of care.
Document assessments, interventions, and case activities accurately in designated systems.
