Qualifications

• Graduation from an accredited School of Nursing and an active Oklahoma RN license.
• Minimum four years of clinical nursing experience in acute care.
• CCM certification preferred; knowledge of managed care and case management.

Responsibilities

• Identify and engage members with complex conditions and elevated health risks.
• Complete member assessments and develop individualized care plans.
• Provide ongoing outreach to monitor progress and update care plans.
• Coordinate medical, behavioral health, pharmacy, and community services.
• Support transitions of care with timely follow-up and discharge coordination.
• Educate members and caregivers on chronic condition management and treatment adherence.
• Address social needs and barriers to care, including transportation and affordability.
• Participate in interdisciplinary care meetings and document care plans.
