Louisiana Blue is seeking a Registered Nurse for care coordination and case management of high-risk members. You will assess needs, plan interventions, and collaborate with providers, payors, and community resources to improve health and cost outcomes.

Requires an active RN license (Louisiana or multi-state with 6-month requirement) and 3+ years of patient care. Louisiana residency preferred; work authorization must be demonstrated without immigration sponsorship.

Qualifications

• Diploma/Associates/Bachelors in nursing required.
• 3 years of direct patient care/clinical experience.
• 2 years in managed care preferred.

Responsibilities

• Assess member status and develop a comprehensive case management plan.
• Coordinate care with providers, payors, and community resources.
• Implement and evaluate case management interventions.
• Advocate for member/family while maintaining privacy and safety.
