Job summary

Louisiana Blue is seeking an experienced RN to provide on-site face-to-face care management at client office sites, ensuring compliance with contractual obligations and coordinating care to improve health outcomes. The position requires a Louisiana RN license, 4 years of clinical care experience, and strong communication and data-analysis skills.

Qualifications

• RN license in Louisiana and ability to obtain/maintain licensure.
• 4 years of clinical care experience.
• 2 years of leadership experience preferred.
• Experience at an offsite facility preferred.
• Knowledge of behavioral interviewing techniques preferred.

Responsibilities

• Develops and implements individualized care plans in collaboration with multidisciplinary teams.
• Coaches employees on care coordination and wellness activities to improve health and outcomes.
• Assesses member status and identifies needs to guide interventions while protecting privacy and confidentiality.
• Coordinates with providers, BC vendors, and internal teams to verify benefits and services.
