Job summary

Louisiana Blue is seeking an RN for care coordination and case management within Population Health. The role focuses on organizing and delivering services to high-risk members, aligning with physician plans, and improving health and cost outcomes.

You will work with Benefits Administration, Medical Directors, and Providers to assess needs, plan interventions, and ensure compliance with regulations. Louisiana residency is preferred and sponsorship is not provided.

Qualifications

• RN licensure in Louisiana (active, unencumbered)
• Experience in managed care is preferred
• Experience with behavioral interviewing techniques is preferred

Responsibilities

• Assess member status and needs to develop a comprehensive case management plan.
• Coordinate resources to meet healthcare goals across the continuum of care.
• Implement and evaluate case management activities to achieve prescribed outcomes.
• Collaborate with members, providers, payors and community resources.
• Utilize behavioral interviewing techniques to inform care decisions.
