The Sr. Program Coordinator, Population Health Programs provides high‑level administrative, logistical, and program coordination support for PCC’s Population Health

Qualifications

• Bachelor’s degree required in public health, health administration, social work, nonprofit management, public administration, business administration, communications, or related field.
• Minimum of three years of progressively responsible experience in program coordination or grant-funded settings.
• Strong written and verbal communication skills and ability to work with diverse stakeholders.

Responsibilities

• Coordinate meetings, minutes, and materials for assigned programs.
• Maintain organized program records, calendars, and administrative tools.
• Track timelines, deliverables, and stakeholder commitments to support progress toward goals.
• Prepare reports, presentations, and invoicing documents for internal and external audiences.
• Communicate with Nexus Board and stakeholders to support follow-up and timely completion of tasks.
• Draft and distribute meeting summaries with owners and timelines.
• Maintain electronic files and program documentation for efficient operations.
• Identify opportunities to improve administrative workflows and tools.
• Other duties as assigned.
