Job summary

P3 Health Partners in Henderson, Nevada, seeks a Program Manager to own the admission and medical expense reduction program across the Red Rock Region. You will synthesize clinical data, create actionable plans, and present them to PCPs to improve follow-up and outcomes.

Hybrid schedule with in-office presence three days per week, travel across the Red Rock Region and occasional trips to the Arizona market. Pay range $100,000–$120,000 based on experience.

Qualifications

• Minimum of three years of experience in primary care.
• Minimum of two years in a managed care or population health environment.
• Experience coordinating care for high-utilization patients preferred.
• Program or project management experience in a healthcare setting preferred.

Responsibilities

• Own the weekly high-risk / frequent flyer interdisciplinary team (IDT) meeting end to end — build the agenda, prepare the patient list, capture decisions and track follow-through.
• Retrieve and synthesize clinical records across multiple affiliate EMR systems to produce a concise case summary for every member under review.
• Translate each IDT decision into a one-page clinical impression and plan, and deliver it to the PCP.
• Contact each practice's designated point of contact by phone to review readmitted members and confirm next steps.
• Direct outreach to cohort members to schedule primary care appointments and connect to P3 programs.
• Coordinate referrals to Council Care, P3 at Home, and hospice/palliative care, tracking from open to close.
• Support advance care planning and end-of-life conversations with the clinical team.
• Collaborate with data/analytics to maintain a weekly roster of repeat admissions and refresh as needed.
• Report program performance against measures like admissions per 1,000, 30-day readmissions, and follow-up rates.
• Document repeatable workflows to extend the program to new affiliates.
