Job summary

Elevance Health is seeking a Population Health Management Pharmacist (Clinical Pharmacist) in Indianapolis, IN, with a hybrid work model (in-office 1–2 days weekly). This role focuses on clinical program quality, appropriate medication use, and supporting cost and quality goals for assigned provider groups.

Key responsibilities include synthesizing clinical data, supporting DURs and therapeutic interventions, educating providers, and staying current with STAR/HEDIS measures to improve outcomes

Qualifications

• Requires BA/BS in Pharmacy.
• Minimum of 2 years of managed care pharmacy (PBM) experience or residency in lieu of work experience.
• Requires a registered pharmacist with active unrestricted state license.
• PharmD degree preferred.

Responsibilities

• Researches and synthesizes detailed clinical data to support cost and quality goals for contracted providers.
• Serves as a clinical resource to pharmacists and contracted providers in areas such as DURs and interventions.
• Prepares information for network physicians using provider tools including Excel.
• Evaluates pharmacy quality program data and helps implement strategies to optimize medication use and manage costs.
• Acts as a subject matter expert in pharmacy-related clinical measures for value-based care agreements.
• Stays current on STAR and HEDIS measures to improve pharmacy quality and outcomes.
• Provides provider education to support clinical measures and reduce costs.
• Builds strong relationships with providers to close feedback loops between pharmacy and providers.
• Serves as pharmacy SME to internal and leadership teams.
