Job summary

Elevance Health in Denver, CO (Hybrid) seeks a Population Health Management Pharmacist to oversee clinical programs and optimize medication use. The role supports cost and quality goals across provider groups, acting as a clinical resource for pharmacists and contracted providers.

You will synthesize data, educate providers on pharmacotherapy measures, and help implement value-based care strategies while maintaining strong collaboration with internal teams.

Qualifications

• Requires BA/BS in Pharmacy.
• Minimum 2 years of managed care pharmacy (PBM) experience.
• Requires a registered pharmacist with an active state license (RPh).
• For associates in Puerto Rico, PR health certificate and PR Law 300 certificate may be required.
• PharmD preferred.
• Experience with Medicare Star Part D measures preferred.
• Proficiency with Microsoft Excel preferred.

Responsibilities

• Researches and synthesizes detailed clinical data related to pharmaceuticals to assist contracted providers in achieving cost and quality goals.
• Serves as a clinical resource to other pharmacists and contracted providers for DURs and clinical support.
• Prepares information for network physicians using provider tools including Excel.
• Evaluate pharmacy quality program data and work with stakeholders to optimize medication use and manage pharmacy spending.
• Build relationships with providers to close the feedback loop between pharmacy and provider.
