Qualifications

• PharmD degree from an accredited school.
• Active pharmacist licensure in state of residence and TX in good standing.
• Advanced training or certification in population health management (preferred).
• Strong analytical and data interpretation skills.
• Experience in population health or managed care pharmacy.

Responsibilities

• Develop, implement, and oversee population health initiatives focused on medication management and adherence.
• Use health data analytics to identify trends and opportunities to improve outcomes.
• Collaborate with providers to optimize medication selection, dosing, and monitoring.
• Educate patients about medications and management of side effects.
• Lead and mentor pharmacy team and support quality improvement initiatives.
